The Unit Selection Form contains a number of sections which must be completed for you to enrol in your course. To assist you in supplying the information requested, an Enrolment Guide has been provided which discusses each section of the Unit Selection Form. These guides have been separated into the different Schools of CHC and also, where applicable, into Undergraduate and Postgraduate guides. You simply need to choose the guide which is relevant to your course. You may print the guide to assist you in completing your Unit Selection Form; alternatively, you may choose to read the contents of the guide on your computer screen.

After you have read your Enrolment Guide, please download and print your Unit Selection Form.

 

When you have completed the Unit Selection Form by providing all of the information as requested, please sign and date the form and return it to CHC at the address indicated.

 

Your Enrolment Guide directs you to the appropriate person or department to contact if you have any questions about completing the Unit Selection Form. This is usually your Course Coordinator.

 

If, after you have contacted your Course Coordinator, you still have questions about the Unit Selection Form, please contact the Student Administration Office.

Enrolling at CHC

To enrol in your course for Semester 2, 2008 you need to complete your Unit Selection Form.
